Required Paperwork

New for 2018 – Parents can complete and submit the forms online or print the packet to fill in and drop off.  Please note the website to fill-in online forms may be a little slow to open.  In order to digitally "sign" the field trip packets all required fields must be filled in.  If a packet has multiple forms that need to be signed you must fill in all field then go back and sign.

Camper Information/Emergency Forms

A Camper Information/Emergency Form is required to be turned in on the first day of camp for every camper.  Please complete a separate form for each camper but this form is good for the whole summer!

Medication

For Campers who require medication during the camp day or for emergencies, please complete a Medication Authorization form and turn it into the Parks & Recreation office by the Wednesday before your camper's first day of camp

New for 2018 – A Camp Nurse will be working with us this summer!  The nurse will not be onsite at camp but will be a resource for our First Aid trained staff and will be checking over all medication paperwork.  Campers will not be allowed to be dropped off with medications paperwork that has not been reviewed by the nurse.

Field Trip Packets

Permission slips are required for the campers to travel by bus to our field trips.  Many of the places that we go to also require their own waivers.  Please complete an entire field trip packet for each of your children, do not put multiple names on the same waiver.
